NYDocSubmit enables residents of New York State to submit required documents for SNAP, HEAP, Temporary Assistance, and Medicaid – avoiding another trip to the local social services district (“district”) office.
This App is available to residents of Albany, Allegany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Clinton, Columbia, Cortland, Delaware, Dutchess, Erie, Essex, Franklin, Fulton, Genesee, Greene, Hamilton, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Montgomery, Niagara, Oneida, Onondaga, Ontario, Orleans, Oswego, Otsego, Putnam, Rensselaer, Rockland, Saratoga, Schoharie, Schuyler, Seneca, St. Lawrence, Steuben, Suffolk, Sullivan, Tioga, Tompkins, Ulster, Warren, Washington, Wayne, Westchester, Wyoming, and Yates Counties at this time. If your district is not listed, check back soon to see if it has been added.
This App is NOT monitored for emergencies. You must contact your district office directly if you need help to address an emergency. DO NOT use this App to submit an initial application for SNAP, HEAP, Temporary Assistance, or Medicaid; to submit a SNAP Interim Report, SNAP Change Report Form, or SNAP Periodic Report; or to submit a recertification application for SNAP, HEAP, or Temporary Assistance. However, you may use NYDocSubmit to submit a Medicaid recertification.
DO NOT submit sensitive information, such as HIV status or domestic violence information and/or addresses that must remain confidential to safeguard you or a household member. If you need to submit such information, or if the App is not available, provide the documents to your district in a manner OTHER THAN through this App, such as through the U.S. Postal Service, in person, kiosk (if available), or fax machine.
Recent changes:
The NYDocSubmit App now allows you to upload documents from your mobile device or photos saved in the photo gallery. The app accepts documents with .jpg, .jpeg, .png, or .pdf file extensions. Additional enhancements were made to improve user experience, including improving image clarity when taking a picture of a document. In addition, performance improvements and bug fixes were applied.
